Fees

Diagnostic Assessment: $250 per therapy hour (53-minutes)

Individual therapy: $150 per therapy hour (53-minutes)

Fees are due at the time of service or the time of invoice. I accept credit card, debit card, HSA card, FSA card, check, and exact-pay cash. A fee of $30 will be assessed for checks that are returned for insufficient funds.

Insurances Accepted

I’m currently in-network with Lyra, BCBS ND, BCBS MN, Preferred One / Aetna, Optum (United Health Care, Medica, UMR), Sanford Health Plan (all except True and employee plans), Health Partners, ND Medicaid, and MN Medicaid.

Please verify your insurance benefits and coverage directly with your insurance prior to your first session so that there are no financial surprises.

Questions to ask your insurance company:

  • Does my health insurance plan include mental health benefits / outpatient psychotherapy?

  • Do I have a deductible that needs to be met? What is it and have I met it yet?

  • Do I have a copay or coinsurance?

  • Does my plan limit how many sessions per calendar year that I can have? What is the limit?

  • Do I need written approval from my primary care physician in order for services to be covered?

NOTE: I do not process secondary insurance claims, so if you are insured by a secondary insurance, then I encourage you to find a provider who can process both of your insurances. If you would still prefer to see me, then you may need to pay the full fee at the time of service. I’m happy to provide you with a superbill / invoice that you can submit to your insurance company to seek reimbursement, however reimbursement is not guaranteed.

Out of Network

I am considered an out-of-network provider for all other insurances. I am happy to provide you with a superbill / invoice at the end of the month that you can submit directly to your insurance for reimbursement. The majority of insurances will cover a portion of my fees, however you will be responsible for the full fee at the time of service. Reimbursement is not guaranteed, so please call your insurance company directly to discuss your out-of-network benefits and fees.

Late Cancellation and Missed Appointment Fee

Please provide at least a 24 hours notice if you need to cancel your appointment. Any appointments that are cancelled within 24-hours or are missed will be charged a fee of $100 that will need to be paid before your next appointment. Appointments where you arrive 15-minutes or more past the scheduled appointment time will automatically be cancelled and will be considered a missed appointment. Please note that insurance does not cover late cancellation/no-show fees.

Good Faith Notice (Effective January 1, 2022)

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.

For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call 701-543-5836.
